Child Care Costs §4062(a)

Child care costs are added to the basic child support obligation. They must be necessary due to employment, job search, or education leading to employment. The costs cannot exceed the amount for care at a licensed level.

Any child care prepayments already made that should be credited against the obligation.

Health Insurance & Medical Costs §4062(a)

Health insurance premiums for the child(ren) and noncovered medical, dental, and prescription expenses are added to the basic obligation. Do NOT include the parent's own health insurance premium here — that is a deduction in Step 2.

Monthly health insurance premium paid to cover the child(ren). This is added to the basic support obligation.

Estimated monthly noncovered medical, surgical, and hospital expenses for the child(ren).

Estimated monthly noncovered dental expenses for the child(ren).

Estimated monthly noncovered prescription medication expenses for the child(ren).

Any health insurance or medical prepayments already made that should be credited.
